Please fill out this form prior to dropping of the animal. NOTE: All required tags, licenses and/or permits as well as a piece of picture ID need to be brought along for verification. A physical signature is also required at drop off.
All animals being dropped off for service must have the appropriate permits and licenses. These have to be physically with the animal during drop off and may have to stay with it. Also photo ID is required during drop off.
Permits for animals found dead can be obtained from Full Service Front Counter Locations.
Note: Permits cannot be issued for Eagles, Snowy Owl, Badger or any red-listed species
